Coverage under the Insurance Plan is available to entertainers and creatives who are listed on the National Registry of Entertainment and Creative Industries Practitioners (e-Registry).  Please sign up to the e-Registry here.

If you are signed up to the e-Registry, proceed to sign up for the Group Life and Personal Accident Insurance benefit by downloading and completing the EB 187 Member Enrolment Form and Health History Questionnaire.

Adjust Information

You may make adjustments to your policy by completing the EB 186 Group Change Request and Beneficiary Update Form.

Make a claim

To make a claim, refer first to the Checklist for Assessment of Group Life Claims.  

Then complete the required form(s):

- Certificate of Death Form

- Group Life Claim Form

- Group Personal Accident Claim Form

- Electronic Funds Transfer Form
